OB Episodes and ACOG

Overview

The clinical and billing time period of a patient's pregnancy in athenaNet is called an OB Episode. The OB Episode is also a mechanism for tracking OB enrollments and information about clinical outcomes. Once saved, OB Episode information is available for reporting.

These OB Episodes (similar to the ACOG Antepartum Record) give providers and care givers information about the patient and their pregnancy as seen in athenaNet. Only female patients can have OB Episodes created for their pregnancy and only one OB Episode at a time may exist. The default configuration is consistent with ACOG standards.

An ACOG Document is a standard report used by OB/Gyn practices that displays patient’s historical information including past pregnancies with the patient’s current or most-recently completed pregnancy information. ACOG documents are generated by a practice user from the OB Episode page in athenaNet when they transmit information. 

The number of fetuses for a pregnancy can be selected within the OB Episode which creates multiple entries in delivery information.  OB Episodes have a concrete start and end date where the provider must close the OB Episode within athenaNet with a valid reason such as pregnancy complete, patient moved etc.  Lab results that are related to the pregnancy can be found through GET /patient/{patientid}/labresults. 

When an OB Episode is ready for transmission to either a hospital or other provider, athenaNet will generate an ACOG document which can be found through GET /patient/{patientid}/documents/acog. If an ACOG document has not been transmitted, the raw HTML of an ACOG document can be found using GET /patient/{patientid}/documents/obepisodes/{obepisodeid}/html.

Note: Past pregnancies can be found through GET /chart/{patientid}/obepisodes with the ShowClosed flag set to True.  The OBEpisodeID from GET /chart/{patientid}/obepisodes can be input to GET /chart/{patientid}/obepisodes/{obepisodeid}.

 

OB Episode Sections:

OB Episode Information

Information on the pregnancy such as the father, domestic partner name, patient blood type and pre-pregnancy weight can be found in the OB Episode Information section.

Menstrual History

Last menstrual period date and other related information can be found through this call.

EDD Calculation

Estimated Due Date or EDD is calculated through different sections in the OB Episode. The Initial EDD Date is the first estimated due date for the pregnancy.

18- 20 Week EDD Update

During the 18 to 20th week of pregnancy, a final EDD is set for the pregnancy and returned through finaledddate.

Genetic Screening and Infection History

A complete list of questions that are available for providers to ask the patient are returned in this section.  Once they have been discussed with the patient, any notes and a true / false response will be returned for the question.

Pregnancy Problem List 

Any problems attributed to the pregnancy will be returned in this section along with any notes about the problem.  The problem name and corresponding Snomed diagnosis code are returned.  Notes that apply to the entire pregnancy, not just one problem, will be returned in the field “listnote”.

Plans/Education

Pregnancy plans that are to be discussed with the patient are separated by trimester.  Once the provider discusses the questions with their patient, they are marked with a Discussed By and Discussed by Date field.  Global questions exist for all athenaNet practices and local questions may also exist for specific practices. Local questions will only be returned in the API if they were added before the OB Episode was started.

Delivery Information

Delivery information will not be available until the delivery date for the baby is set in athenaNet. Weeks Gestation is returned in the form of 39.2 weeks. This denotes 39 weeks and 2 days, not 39.2 weeks.

Prenatal Flowsheets

Prenatal flowsheets track the changes of the patient and the baby for each visit they have with providers.  Prenatal flowsheets are not returned in the OB Episode API currently, but will be available after our August release.

Note:  Older OB Episodes may not contain all fields that exist today as new requirements were added after the creation of the OB Episode.  Often only delivery information will be available for past pregnancies that are input for historical purposes.  OB Episodes created after the fields were added to athenaNet will return a full set of results, however.

 

APIs

Note: Since underlying OB Episode is updated after each patient visit, this API will always return latest information. acogid is used only to determine specific pregnancy from a patient’s chart.
